Phoenix introduced the TheraBand® Active Care System to each of its outpatient facilities several years ago. Endorsed by the APTA, these rehabilitation and wellness products are useful for therapeutic exercise, neuromuscular re-education, and functional activities.
Packaged for consumers and designed to transfer a clinic program to Transitional Care program, the TheraBand® Active Care System gives healthcare providers new options for transitional care exercise programs as well as expanding current clinical procedures. Additionally, these products come with comprehensive illustrated exercise instructions for improved ease of use by patients in TheraBand® physical therapy.
Patient empowerment, motivation, and active participation in Transitional Care exercise programs that complement in-clinic care are all critical topics for today’s healthcare providers. They promote better outcomes and a transition to a healthier lifestyle after recovery.

Transitional Care Therapy
Transitional Care Therapy services allow patients to heal in the comfort and convenience of their own home. Transitional care therapy is the solution if a patient’s health or transportation issues make it difficult to attend an outpatient clinic.
Compact equipment allows us to perform a wide range of modalities in the home including ultrasound, electric stimulation and near infrared treatment for wound care & pain management.
Transitional Care Therapy Treatments
Phoenix Transitional Care Therapy delivers the following services and treatments to patients in the comfort of their home, as well as residents of assisted living facilities:
Physical Therapy: Works to obtain normal movement by focusing on balance, endurance, gait, transfers, strength, range of motion and reduction of pain.
Occupational Therapy: Improves the use of upper extremities while dealing with functional daily skills such as feeding, bathing, grooming and toileting.
Lymphedema Therapy:
- Manual lymph drainage
- Compression therapy
- Decongestive remedial exercises
- Wound care and skin care
- (ABI) Ankle-Brachial Index
- Venous Refill Testing
Transitional Care Therapy also provides the following Special Services:
Home Assessment: Evaluation for modifications needed for safety and completion of Activities of Daily Living (ADLs)
Durable Medical Equipment Assessment: Wheelchair evaluation, ramp evaluation, etc.
